Preservation of Residual Radicular Pulp: A Biologically Guided Vital Pulp Therapy Protocol for Immature Teeth with Fractured Dens Evaginatus and Apical Periodontitis

Abstract
Background
Immature permanent teeth with fractured dens evaginatus frequently develop apical periodontitis before completion of root formation. These teeth are commonly managed as completely necrotic, although viable radicular pulp may persist despite periapical disease. Recognition of residual radicular pulp may permit a more conservative biologically based treatment strategy.
Objective
To describe a biologically guided vital pulp therapy protocol based on preservation of residual radicular pulp and to evaluate its clinical application in a consecutive series of immature premolars with fractured dens evaginatus and apical periodontitis.
