Abstract


A 40-year-old medically healthy woman presented with tenderness to percussion of tooth 28 (mandibular right first premolar) and an associated periapical radiolucency. The crown was clinically intact. There was no caries, restoration, discoloration, visible crack, or history of acute dental trauma. Cold testing was negative, and cone-beam computed tomography (CBCT) demonstrated pulp canal obliteration extending through the middle third of the root. A patent canal lumen was identified apical to the calcified segment.
